Headers and footers do not work in pdf-utils extension

Hello, please help me
I created a watermark table and attached it to the second page of .pdf doc

This is how I configured the watermark:

This is how it looked like in the .pdf:

Whenever I display Headers/footer for template, it wont work. I need UID to be in footer on every page of .pdf

Could you please point it out why footer is not working?
I tried to construct footer file for all of templates the same, but it wont work as well.

Please don't combine pdf utils and native chrome headers. Use pdf utils merge operation to merge the header instead.
