Refresh index / table of content in docx template

Hi, is there any way to refresh an index / TOC in defined in a docx template at render time?

The openxml that drives the docx format doesn't have a feature for the automatic TOC update.
Our engine has no clue on which page is the particular heading so we cannot do this for you.

It seems that the only way to do this is by using macro, but then you probably need a macro-enabled document.
