Your icon for the report shows PDF below the form. I do not have that option to create a pdfform

Just another update. I was able to export your playground sample and import into my profile and even though it does not show the same icon it still works as per your playground sample.
I think my issue was that I was not able to create this example from scratch. But at least I can extrapolate from your sample.
